There are a few ways to add text above an image in a Google Doc. Here's how to insert an image in the background so you can type over it in Google Docs:
Open a new document and go to Insert > Picture, then choose an image. You can upload a file, take a photo, or search for a photo online.
Select the image and then select the three dots under the image. In the pop-up window, choose All Image Options.
