It is a part of the Saint's corpse along with 9 other parts, which Spoilers Alert belongs to Jesus Christ in the manga. You can use it to get Dirty Deeds Done Dirt Cheap (D4C). Speedrunning D4C: Love Train in a Universal Time
To get Heart of the Saint you need to go to the Devil's Palm, a desert area after the Forest. The only way to get HotS is to dig in the Devil's Palm. You have a 4% chance of finding it. One tip I would recommend is to go Standless and when your health bar turns red press the H-Key on your keyboard. This will give you some more time to dig in the desert while you gain some health. HotS is an item that looks like a human heart.
The first thing you need to do is Standless and use Heart of the Saint. Once you do that, you will get Stand D4C (Dirty Deeds Done Dirt Cheap).
