The Reverse-Hand Sword is located in front of a grave just before the Scorched Ruins Site of Grace in the Realm of Shadow. To get there, travel to the Scorched Ruins, where you'll see the marked grave. Go to the grave and in front of it you'll find the Reverse-Hand Sword.
If you haven’t discovered the Scorched Ruins Site of Grace yet, head north from the Gravesite Plains Site of Grace. This is the first one you’ll discover once you start the DLC. As you head north, you’ll see a road and some broken structures. The Scorched Ruins site of grace is located to the right of this road.
The unique feature of the Reverse-Hand Sword is that it becomes two swords when wielded in both hands, but that can be a pain, especially if you come back to the game after a while, like I did. So to wield it with two hands, equip the Reverse-Hand Sword in one of your hands, hold the interact button, and then press the attack button on the empty hand.
