Three Swollen Grapes can be found in Midra's Manse, a dungeon in the southern part of the Abyssal Woods in Shadow of the Erdtree. If you want more, however, you'll have to defeat the Aging Untouchables, the fearsome enemies that will stalk you if they spot you.
The most guaranteed source of Swollen Grapes in Shadow of the Erdtree is Midra's Manse, home to Midra, Lord of the Frenzied Flame. These three—one near the entrance, two by the fireplace in the dining room—can be plucked from corpses without anything attached.
To get more, you’ll need some tactics and a whole lot of timing. Despite all the warnings, you can actually defeat the Aging Untouchables. To do so, let one spot you and go into chase mode. You can either try to dodge the teleport or let them lie down and miss the first attack. Then, when they swing their wand to hit you with their deadly command grab, you have to parry. The timing is weird, because they can vary their approach, but if you parry as soon as the wand goes up, you should be in a good spot. Then, riposte until they go down.
