To get to the Derceto Asylum’s infirmary and operating rooms, you’ll first need to find a Stairwell Key in Dr. Gray’s office. It’s in the right drawer of his desk. But that’s not all. All roads to the Grand Parlor and the Stairwell are blocked off. Things can get confusing, so here’s how to solve the Jeremy X-ray puzzle in Alone in the Dark.
Instead, you'll need to head to the living room, where you'll find MacCarfey. After he falls to the ground in the cutscene, he drops the 1st Floor Hall Key, which you can pick up. I like this sequence in Emily's playthrough, because MacCarfey just lies there unconscious on the ground.
