Here's how to do it via a desktop web browser, YouTube.com on the mobile web, and the YouTube app for iOS and Android.

Go to YouTube.com in a web browser on your desktop or mobile device, or open the iOS/Android app on your mobile device and sign in with your Google/YouTube account.

Select Subscriptions in the left pane of a browser or in the lower-right corner of the app.
