On April 15th, a player named TheCrowsPalace shared on the Baldur's Gate 3 subreddit that Alfira would be your companion. It seems that there are remnants in the code that indicate she was a companion.

You meet this lovely Tiefling early in Act One, in Emerald Grove, and you’ll interact with her throughout the other two parts of the story (unless she tragically dies). From your first interaction where you help Alfira finish the song she’s been working on, it’s clear that she has more personality than the other NPCs who are there to advance the story or simply be quest-givers. Alfira is kind and curious, but most importantly, she’s a Bard. If you want a Bard in your party, you’ll have to make one yourself.

TheCrowsPalace found this while interacting with Mayrina in Act Three after meeting Auntie Ethel due to the Show Approval Ratings in the Dialogue Choices mod. This mod shows you which companions approve or disapprove of your decision and how much rating you can gain or lose. This mod is useful if you seduce a companion and want to trigger specific romance scenes early. For example, you can start your romance with Astarion when you reach 40 approval with him, and you don't even need to have a party.
