Joker 2 Folie a Deux is terrible
Fans have been wondering what the guards did to Arthur Fleck's Joker in Joker 2: Folie a Deux. The question isn't all that surprising, considering that a significant portion of the film's running time sees Joaquin Phoenix's title character spend time in prison and in court, detailing his experiences with prison conditions and his relationships with the guards and fellow inmates. So, did the guards beat and abuse Arthur? Did they injure or kill him?

The guards, led by Brendan Gleeson's Jackie Sullivan, take Arthur to the shower room at Arkham State Hospital and brutally beat him, taking him back to his cell half-naked. This is in retaliation for Arthur's revelation to the court about their unfair and cruel treatment of him in prison. They also kill his fellow inmate and friend Ricky by strangling him after he confronts them about Arthur's mistreatment.
